Per QUANTEC recommendations (Bhandare et al. IJROBP 2010), to minimize the risk of hearing loss the mean cochlea dose (whole organ) should be kept ≤45Gy, or more conservatively ≤35Gy, for patients undergoing standard fractionation. This corresponds to a risk of sensorineural hearing loss of < 30%.
For patients undergoing SRS for vestibular schwanomma, the prescription dose should be limited to 12-14 Gy for hearing preservation.
